FIRST APPOINTMENT

Prior to your Appointment Date:
Upon booking appointment, we will send an email invitation to the patient portal. You can access testing results, and medical records via this portal. Seven days prior to your appointment, you will receive a text message to check-in for your appointment. This is completed via text message and will take you through your medical history and allow you to sign consents before arriving to the office. Completing this prior to your appointment saves SIGNIFICANT time during the checkin process and is recommended for all patients!

Upon Arrival:
A member of the front desk staff will greet you to gather your photo-id, along with any insurance information and help you to complete any remaining patient forms that need signing.

What to Bring:
● Photo ID
● Current Insurance ID
● Valid Credit Card-we REQUIRE to keep a credit card on file for all patients regardless of coverage unless you are a self-paying, or cosmetic only patient.
● Updated list of Medications and Allergies
● Any products or prescriptions that you are currently using that may be relevant to the visit
● Pertinent Medical Records from prior dermatologist

Appointment for Minors:
All patients that are minors must be accompanied by a parent or legal guardian for the first visit. If the parent or legal guardian would like the minor to be accompanied by another party for the subsequent visits, we have a consent form that must be signed.
